2 OHP Drs Brinksman, Conlon, Manley, Saunders, Hull, & Martins inspection report 05 June 2019; The practice offered extended appointments to patients with dementia and mild cognitive impairment, a dedicated fortnightly GP lead supported clinic was held with local support workers to ensure patients' care was more effectively coordinated and that patients and their families received appropriate support. The staff, informed us, the clinics enabled the clinical team to get to know and support their complicated patients at the practice better and to provide appropriate support for them. Well links with local support services were established through these clinics.
